This enrolment form is the first step toward building the personalised foundations your child needs to thrive. By understanding their unique developmental milestones and your family's preferences, we can design a specialised 1:1 program that empowers their growth for school and beyond. All information shared here is strictly confidential and used solely to prepare for your child's success.

Program & Session Choices
I can offer flexible times outside of the designated times I am running sessions to better meet your needs - At the moment, Bookings can be made from 4:00pm - 5:00pm Friday evenings or from 9:00am - 3:00pm Saturdays: Contact me to see if we can work out better times and dates for you and your family if you are interested in the care and education I provide. I advocate for Early Years Education, but also to work with families to make scheduled times that work for them!
Final Step & Consent
Once the Enrolment Form Sections are completed and reviewed by Jane, she will get in contact with you regarding the rest of the booking processes as well as lessons and packages.
There will also be one last form sent to you to fill out via Email in regards to the finalisation of our Agreements and further Consents.
